Our Value

Let us be an extension of your family

To provide comfort, compassion and extraordinary care to patients and families experiencing a need for skilled nursing and multidimensional home care.

Our Home health care services cater to individuals with both short-term and long-term medical needs as prescribed. While it's commonly recommended post-hospital discharge, doctors may also prescribe home health care to manage chronic conditions like Parkinson’s, COPD, Alzheimer’s, traumatic brain injuries, or diabetes. Medicare and many private insurance plans typically cover home health care when deemed medically necessary by a physician. In instances where self-payment is necessary, AHHC's flexibility simplifies budgeting, allowing individuals to pay solely for the services required and only for the duration needed. This approach often renders home health care more cost-effective than many long-term care or rehabilitative facilities.

Vision

In delivering transformative care, we earn the trust and respect of our clients, elevate the role of the caregiver and build meaningful relationships within the communities we serve.

Mission

Our mission at AHHC is to serve our patients with high ethical standards, principles and heart. We commit ourselves to providing our patients with the best in home health care possible.

AHHC offers the patient and family the most expert and nurturing care during all phases of a patient's healthcare journey. At AHHC we are "Always There" and provide the patient and family with choices across the continuum of care.

AHHC employs registered nurses, licensed practical nurses, medical social workers, and skilled physical, occupational, and speech therapists. Each team member undergoes a rigorous hiring process, which entails background and reference checks. They are also required to engage in continuous training to stay abreast of the latest advancements in healthcare.

Although a significant portion of our home health care clientele does transition to us following a hospital discharge, it's not a prerequisite. We routinely receive referrals from primary care physicians and specialists as well.

AHHC serves individuals across all demographics. Whether it's an adult recuperating after a significant car accident, a senior navigating diabetes management, or a college student facing a chronic medical condition, our skilled nursing and therapy services deliver tailored care within the familiar environment of home.

Home care nurses possess specialized skills in particular medical tasks. While they can aid with some activities of daily living (ADLs) during their visits, their primary focus will be on addressing your medical requirements. If you need consistent help with tasks beyond their usual responsibilities, AHHC can complement your care with in-home health aide services.

Choosing what to do after a hospital stay is a personal choice, and the optimal decision can vary for numerous reasons. Consider your objectives when making this decision. If your goals involve aging in your own home, home health care could be the ideal option for you. Although many facilities offer excellent care, it's important to note that only home health care offers personalized, fully customized care within the comfort and privacy of your own home. For many individuals, this translates to increased time with family and reduced financial strain, particularly when managing a chronic medical condition.

At AHHC, we grasp the significance of maintaining consistent care, and the sole method to guarantee our clients receive this is by connecting with their existing care providers. During your initial evaluation, our care manager will reach out to your doctor, hospital discharge planner, and social worker to seek recommendations and insights. Together, in collaboration with you and your family, we will utilize this information to craft a suitable care plan that fulfills all your requirements.

At AHHC, our aim is to facilitate your relaxation and concentration on your recuperation. We take charge of scheduling multiple workers, ensuring that the professionals visiting your home arrive at a time that suits you. As our client, if you ever need to modify the schedule or handle an emergency, our care managers are always available to help.

Yes. AHHC's expert nurses offer family education. There are various tasks that your loved ones can support with, and there are even some that you might be able to manage on your own.

AHHC never requires our customers to sign a contract. We also do not charge any fees for canceling your services.
